plan At Cape Cod Healthcare, the rehabilitation program is tailored
to your individual needs. To do this, rehab therapists work with healthcare team
members to create an individual care plan for each patient.
“Care Maps provide a standardized approach to patient care,” said Richard K.

“They ensure all appropriate steps are taken after surgery.”
The Care Map may include any number of rehabilitation programs.
“The continuum of care is the major focus and it is a real strength of Cape Cod
Healthcare,” said Pat Wolfe, PT, MS, director of orthopedic services and
rehabilitation services at Cape Cod Healthcare. “We work with nursing,
rehabilitation, and healthcare team members at home health, and skilled nursing
and rehabilitation facilities to establish and collaborate on Care Maps. The
Care Maps ensure that protocols and practice guidelines are clear. Documentation
is online and communication is readily available.”
A Team
Effort Along with an array of therapy services, Cape Cod
Healthcare offers an integrative approach to patient care. The rehabilitation
team includes physicians, physical and occupational therapists, speech/language
pathologists, cardiac nurses, exercise physiologists, and therapy assistants.
The team collaborates with surgeons and other clinicians to help patients
overcome their obstacles to independence and improve quality of life.
“By working with the rehabilitation team, I can closely monitor
patients’ progress from day one,” said Paul Dimond, MD, a board-certified
orthopedic surgeon on staff at Falmouth Hospital. “It also allows me to bond with
patients and understand their conditions in greater detail, which ultimately results
